Below is some of the text about Colleen Conaway from the PDF above.
Melinda Martin said below that she first heard about Colleen Conaway on Oct 19, 2009.
The death of Colleen Conaway was reported in this thread on Oct 14, 2009. [

Was the seminar attendee who witnessed Colleen's death, but did not give his address or information as he didn't want to stop playing homeless...AF Grant, Greg Hartle, or another person?
It sounds like that person did not want to be connected with the death.
Is it normal police policy to NOT take the " address or information" of a witness to a death? If you are a witness to a fist-fight or car accident, the police write down your information from your ID.
So if you are a witness to a death, they don't bother to check someone's ID to verify their identity, in case more interviews are needed?
Did it not occur to anyone it might not be a "normal suicide", and that someone else could have been going on in that seminar in the mall?
Why is it, than when people die and get seriously injured during a James Ray seminar in Arizona, there is a full investigation?
And why is it, when a person dies during a James Ray seminar in San Diego California, nothing happens?
Colleen Conaway died DURING a seminar at the Horton Plaza mall.
Was the mall even notified that this death occurred during a James Ray seminar in their mall? Or if Sedona had not happened, one imagines James Ray would have taken another group of "homeless" people to the Horton Plaza for the next seminar?
The entire thing really is outrageous. How can it be, that a seminar leader can take people on a seminar "exercise" and one person dies, and that is not negligent, especially considering how it was covered up?
Isn't that as negligent as the sweat-lodge? What's the difference?
What if "only" 1 person died in Sedona, and the seminar Staff called it a suicide, as someone fell off a cliff?
You get the sense that in Arizona they would do a full investigation, but in Sad Diego California, you are on your own if you die during a seminar.
There should have been an full investigation in San Diego, and if there was, where are the reports like the one below from Arizona?
It doesn't add up.
____________QUOTE_______________
Detectives learned the following information from different sources, including, Amy Hall
(former James Ray employee), Melinda Martin, and family members of Colleen Conaway.
On 07/25/09 Colleen Conaway participated in a James Ray event in San Diego. During this
event James Ray and his staff would make the participants look like homeless people, take
their identification, and drop them off in San Diego. The purpose of the event was for the
participants to pretend they were homeless. Amy Hall, a James Ray employee who resigned
after the sweat lodge incident, told us some participants would actually dig through garbage to find something to eat after only a few hours. Colleen Conaway went inside the Horton Plaza mall, went up to the third story, and jumped to her death. Colleen's sister told me Colleen was not suicidal and the friends and family couldn't understand why she would do this. Colleen was dressed as a homeless person and didn't have identification.
Another participant witnessed the suicide but would not give his address or information to law enforcement because he didn't want to come out of character. Colleen did not return to the bus and James Ray's staff notified mall security. Mall security advised James Ray's staff of the suicide. It was eventually determined that the suicide victim was Colleen.
Melinda remembered a staff meeting about the event. The staff was not told Colleen was dead. James Ray told staff members that if anyone asked them where Colleen was they were to say that Colleen left and wasn't corning back. It is unknown which staff members knew of Colleen's death. Staff members Amy Hall and Melinda Martin did not know about the July death until after the deaths at the sweat lodge. On 10/20/09 Melinda Martin called Det. Edgerton to say she had just found out 10/19/09 that Colleen had died during the San Diego event.
