As far as the book on Werner Erhard, it appears to be out of print, and so the author is not making any money off it.
There is no ebook available for sale either.

That book will probably never go back into print, due to the publishers fear of frivolous lawsuits from Werner Erhard, which will make the book lose money. So the libel-chill tactics of Werner Erhard appear to have worked.

There are still a number of secondhand copies of the book available on Amazon. The 'hot' and 'cold' reviews on this site are also very interesting...
