Informed Consent Release Forms

Dear Forum Members,
It occurred to me that in all my years in realm of professional development, executive education, I have yet to see an "informed consent" released used with clients.
At most I have seen simple contracts that disclose to coaching participants what information can be shared with their organizations. That's it.
So, we are really talking about a whole other animal when Landmark sells its programs to organizations, or when organizations subsidize their employees for costs incurred in Landmark participation.
I am wondering what corporate compliance departments think of Landmark waivers, and who signs off on these.
Does anyone have any experience with this, any knowledge of how Landmark, or similar entities deal with corporate accounts?
